I thought I had filled out the form correctly and took some time doing so, but was told that the recipient address was not on the form and thus the item could not be sent to them. The item is really heavy, so this was a significant problem.

A portion of the unit arrives wet but thats because they test it before shipping. I called customer service and immediately spoke to a person, Ms Michelle was very helpful and had the answers I needed. Very nice RO system superbly packaged with excellent instructions. There is also an installation video on youtube that you should watch.
